Birth control: Birth control pill
, Hormone patch, Diaphragm, male and female Condoms, Spermicide, Intrauterine Device (IUD), Cervical cap,
The device is placed under the skin, such as Nexplanon or Implanon
Methods to prevent pregnancy permanently (tubal ligations or Essure)

Follow-up of family planning visits related to your birth control method.

This program pays only for the services listed above. If health problems such as diabetes or cancer are found, you will be referred to your doctor or clinic who can treat you. You may have to pay for the additional service.
